All insurance plans are a contract between the patient and the insurance company, and all plans are different. We have no influence over the treatments covered or the percentages covered. Please be familiar with your plan, and ask our front desk staff if you have questions regarding coverage and we will do our best to help.
We will provide you a treatment plan with a breakdown of costs before treatment begins. We will gladly submit this treatment plan to your insurance company for treatment preauthorization and specific costs, but you are personally responsible for your account and coverage details.
